You can sign the Charter for Employers who are Positive About Mental Health by completing the online form below.
Please read before submitting online form.
The decision to sign the Charter must be made by CEO/MD/Owner/Head of Personnel or HR or someone of similar standing within the organisation. Please enter this person's name, position and e-mail address in the boxes marked 'Name of Signatory'. Please also enter details of a contact person for our response and further information.
Submitting this form confirms that your organisation
- Has decided to sign the Charter for Employers who are Positive About Mental Health
- Agrees to undertake to use the MINDFUL EMPLOYER name and logo correctly
- Agrees to pay Administration Fees when requested
- Understands that your commitment to the Charter will be initially reviewed in 2 years time and every 3 years thereafter and
- Will inform Workways if you no longer wish to be a signatory.
The date you submit this form will be used as the date of signatory to be used for Review purposes.
You will receive an acknowledgement and be listed on our website within 5 working days. If you have requested an invoice for the appropriate New Signatory Administration Fee, this will be sent to the Contact Person shown below. Please use the Message/Comment box to advise of different invoice address if applicable. Currently, we are unable to accept online or credit card payments.
Whichever method you choose to pay the fee, your printed Charters, sample publications, the MINDFUL EMPLOYER logo and other information will be sent once payment has been received.
